Medical Records
We support patients who are seen in our STD, Tuberculosis and refugee clinics located in our Public Health Clinic building at 1645 E. Roosevelt Street. If you require records from one of our other Public Health clinics (such as WIC, Childhood Immunizations or Vital Registration), please contact their office or visit their webpage for assistance.
HIPAA Compliance

Records Releases
If you would like to receive a paper copy of your records, or if you would like us to send your medical records to your employer, doctor or other facility, we will need a completed Release of Information form. Release forms are available for download (English | Spanish) or by calling (602) 506-6018. Release forms are also available at the Medical Records office, located at 1645 E. Roosevelt Street in Phoenix. Office hours are Monday, Tuesday, Thursday & Friday, 7:30 AM to 5:00 PM and Wednesdays 9:00 AM to 5:00 PM.
You can submit your request in three ways:
- Walk-ins for copies of medical records are accepted Monday, Tuesday, Thursday & Friday between 7:30 AM to 5:00 PM and Wednesdays 9:00 AM to 5:00 PM. Please bring a valid government-issued photo ID card that includes your name and date of birth.
- Completed release forms can be mailed to our Medical Records office at 1645 E. Roosevelt St, Phoenix, AZ 85006.*
- Completed release forms can be faxed to (602) 506-5079.*
*Mailed and faxed forms must include the completed notary section on the form and be accompanied by a legible copy (front and back) of your photo ID.
**Please note: If the name and date of birth on your photo ID do not match the name and date of birth on your record, you will not be able to get copies of your medical record or lab results**
It may take up to 5 business days to for the request to be completed as all records from services provided prior to 2013 are in storage and must be retrieved.
Patient Portal
If you have signed up for the Patient Portal during your last visit and are having trouble, please read the FAQs for assistance or you can call 602-506-6018 for assistance. Please note: we cannot reset passwords over the phone, you must come in with a valid photo ID to obtain a new password for the portal.
