FAQs
1. How do I get access to my Patient Portal?
If you are at least 18 years old and you are a patient in our STD or Tuberculosis clinic, you can sign up for a Patient Portal account in Medical Records. You will need to have a valid photo ID to complete the Portal activation process.
2. What sort of information is available on the Patient Portal?
Your full medical record (visit notes, lab results, x-ray results, appointment history, medication information, etc.) is available to you on the Patient Portal. You can also send secure messages to the MCDPH staff regarding your healthcare (please note that these messages will become a part of your medical record).
3. If I don’t have a Patient Portal account, can I still get my medical records?
You can still get any part of your medical record on paper by coming in person to the Medical Records office at 1645 E. Roosevelt St and presenting a valid photo ID. If you were seen in the STD Clinic, you may also call the Results Line and provide your 5-digit security code to get your results over the phone.
4. Am I allowed to set up a Patient Portal for my children, spouse or partner?
Due to the confidential nature of a medical record and our commitment to patient confidentiality, our policies do not allow Patient Portals to be set up by anyone except the patient.
5. I never received an email with my username and password, what do I do?
The message would have been sent by MCDPHPortal. Please remember to check your Junk and Trash email messages to ensure that the message did not get lost.
Go to the Portal Website at https://mycw80.ecwcloud.com/portal10519/jsp/100mp/login.jsp and click “Can’t access your account”. You will be able to get your username and/or password from there. Please call Public Health’s Medical Records office at 602-506-6018 during normal business hours for additional assistance.
6. I deleted the email with my username and password. Can you send it to me again?
We are unable to resend Portal activation messages. The message is system generated and is only sent once. After that, you will need to return to the Public Health Medical Records office located at 1645 E. Roosevelt, Phoenix, with a valid photo ID to re-establish your account.
7. I tried to log in to my Patient Portal and it says I am locked out. Can you unlock my account?
The only way to unlock your Patient Portal account is to come in person to the Medical Records Office. You will be required to show a valid picture ID.
8. Is there an app for my phone that I can use?
Yes! Please download the Healow App available on iTunes and Google Play. The Practice Code is HBIJBA.
