About the Community Health Survey

Every three years, Maricopa County Department of Public Health, in collaboration with health care and community partners, conducts a Community Health Survey to develop a deeper understanding from residents about issues that relate to quality of life, availability of services, physical and mental health, and more. 

This survey is a key part of a more comprehensive Community Health Assessment process used to identify the most important problems that can be addressed through community action. Past community health surveys have already been instrumental in drawing attention to emerging health issues, such as improving healthy food options in low-access communities.

By collecting information about health at the community level, the Maricopa County Community Health Survey aims to:

  • represent health and quality of life data from a diverse mix of residents representing every community in Maricopa County; and   
  • support public health and community-based efforts to evaluate existing initiatives and to develop new ones suited to their communities

All community members ages 14 and up from every region of the Valley are encouraged to take the survey. Answers are completely confidential and no personal information is required. The survey is available in English and Spanish and takes approximately 10-15 minutes to complete.  For more information, or to obtain the survey in an alternative format, please submit a request for an alternative format.
