1. Where do I get a Disinterment Reinternment form?: For the Central Phoenix location only, contact the Medical Certification and Disposition Permit Unit. Find the Central Phoenix location.

3. Do I need any supporting documentation?: If the death occurred prior to 2009 you will need to submit a certified copy of the death certificate in addition to the permit request.
